New Graduate Packet

At 18, your child becomes a legal adult and obtains certain rights. As a result, you as the child’s parent lose certain rights. The documents in the New Graduate Packet allow you to protect your child’s interests if your child ever becomes incapacitated and cannot make their own decisions.

A Financial Power of Attorney lets your 18+ year old child appoint someone to make important financial decisions for them, such as banking, insurance, and tax decisions. If your child become incapacitated (unable to make decisions for themselves), the Financial Power of Attorney helps avoid an expensive, lengthy court process to appoint someone to manage their affairs.

A Medical Power of Attorney helps avoid uncertainty and delay by appointing someone to make medical decisions for your 18+ year old child if they are unable to make them for themselves.

A HIPAA Disclosure Authorization works to ensure the people your child trusts can make medical decisions on their behalf and are able to stay informed about their condition, treatment, and prognosis by allowing access to confidential medical information.

A Directive to Physicians allows your 18+ year old child to plan ahead by instructing their doctor on when to administer, withhold, or withdraw life-sustaining treatment if they are diagnosed with a terminal or irreversible condition and can no longer make decisions on their own.
